How to Submit For a Person to Receive Cards
 
** Submission Limit for 2017 - We have a limit TOTAL of THREE (3) Submissions per individual submitting names. This year we have limit of ONE submission per Mailing Address with the exception of Group Homes and Nursing Homes. This is to help prevent abuse of the program. 
‚ÄčAny person not following this guideline will have ALL submissions removed.

SPECIAL NOTE IF SUBMITTING A MINOR'S NAME:

If you are submitting the name of a person or persons under the age of 18 years old, we must have the name of the parents or guardian. There will be a place on the form for that. A letter will be generated to the parents to explain to them why their child or children will be receiving cards and who submitted their child's name. If this box is not filled out, the submission will be deleted.

**All names MUST be submitted by November 27th, 2017 at 4:00pm central time to be included in the list for this years project. No submissions will be taken after that date.

Please make sure that you fill the form out completely and correctly. If you DO NOT then your submission CAN NOT be accepted and no other notification will be given**
